本文件是為了要評估電子產品中所可能使用到的衝突礦物 The purpose of this document is to evaluate the use of conflict minerals in LITE-ON IT products.

衝突礦物(金屬)，是指金（Au），鉭（Ta），鎢（W），鈷（Co），鈀（Pd）和錫（Sn）等金屬，來自於剛果民主共和國或其鄰近國家非政府軍事團體或非法軍事派別所控制衝突地區的礦區。當地軍事團體取得的非法採礦利潤是從公民中盜竊得來的，而且在剛果民主共和國東部造成侵犯人權及環境惡化。
Conflict minerals (metals) are metals such as gold (Au), tantalum (Ta), tungsten (W), cobalt (Co), palladium (Pd) and tin (Sn) derived from minerals being sourced from mines in Democratic Republic of Congo (DRC) conflict areas or surrounding countries which are controlled by non-government military groups, or unlawful military factions. Illegal mining profits by local military groups in the eastern DRC are contributing to human rights abuses, environmental degradation, and theft from the countrys citizens.

This form should be completed for all products from a single plant providing products or components to LITE-ON IT Corporation. If more than one plant is used to provide product to LITE-ON IT, please complete a form for each additional plant.

An appropriate authority of the company should complete the certification by completing the "Certification" section below, answer questions (two) and (three), and email completed forms to SQM.
